This matchup turns on whether Dr. Mario gets to turn pills into point-blank pressure, or whether Shulk keeps pushing from outside that entry route with sword range before the close-range game even begins.
Dr. Mario’s down tilt, aerials, Up B, and Down B are brutally rewarding once he gets in, but his mobility and reach remain short, so Shulk is more stable when he drifts around pill drop points with base form and Speed, then tipper-pokes with Forward Tilt, Forward Air, and Neutral Air.
The table shows that Jab 1-3, Forward Tilt, Up Tilt, Down Tilt, Dash Attack, every smash attack, Forward Air, Neutral B, Side B, and Down B all lose to Shulk’s Grab (10f) or Up B (10f), while Neutral Air, Back Air, Up Air, and Down Air leave almost no direct punish, so those air buttons should just shift the scramble into the next landing or ledge guess.
For kills, it is more repeatable to keep Dr. Mario pinned at ledge with Forward Tilt, Forward Air, Up Air, and Up B, then let Smash convert that one ledge sequence, than to brawl in center where one Up B or Down B swing can reverse the stock.

Win Condition Checklist

  • Rotate between base movement and Speed so you pressure from just outside pill fall space and force Dr. Mario to approach through your sword instead of into your shield.
  • After blocking Jab 1-3, Forward Tilt, Up Tilt, Down Tilt, Dash Attack, any smash attack, Forward Air, Neutral B, Side B, or Down B, answer first with Grab (10f) or Up B (10f) and cut the close-range sequence immediately.
  • After blocked Neutral Air, Back Air, Up Air, or Down Air, do not force a punish; shift your position with Speed and cover the next landing or ledge route with Forward Tilt or Up Air instead.
  • At kill percent, mix in Shield to lower the chance of losing to Up B, Down B, back air, or up smash, and avoid lingering in Buster or Smash for grounded trades.
  • Once Dr. Mario is offstage, keep stage control and split his Up B rise plus Down B stall between Forward Air, Up Air, and Up B rather than over-chasing.

Actions to Avoid

  • Do not jump over every pill on autopilot and give Dr. Mario the exact anti-air and landing trap he wants.
  • Do not force shield punishes on every blocked Neutral Air, Back Air, Up Air, or Down Air and hand the next close-range opening back when those punishes whiff.
  • Do not brawl in center while staying in Buster or Smash and widen Dr. Mario’s reversal windows into Up B, Down B, or smash attacks.
  • Do not chase too deep offstage into his panic buttons when Shulk’s sword already covers the recovery more safely from ledge.
